Does metal clad cable need to be sleeved passing through a cored hole through concrete?. I see Romex does per code at least 6" above floor but I didn't see anything about mc cable

Seeing in your profile that you are using the 2011 NEC, you'll find 330.10(A)(10) & (11) Uses Permitted and 330.12 Uses Not Permitted will help.
 
IMO. You might well be in a AHJ call area. I can't see a sleeve would be directly required, but if it were judged that the conditions "subjected the cable to physical damage" then 330.12 would come into play.

For us if it comes up inside a wall no sleaze needed.

If it comes up through the floor exposed than a sleeve is needed.

I agree that would be the proper rule to follow but to you see it as "Code required" ?
(am I overlooking something)
