Meter Main with Breaker Panel Main

What is the proper hook up for a 200 amp service with a 200 amp meter main breaker? The service from the meter main breaker is 4/0 SEU going into the 200 amp breaker panel. The breaker panel also has a 200 amp main breaker. Can both of these breakers be used? I assume the meter main can be used to protect the SEU while the main breaker in the panel will act as normal. If both are used does the main breaker panel need to be treated as a sub panel, and the neutral and grounds separated?
 
The short answer is Yes- separate the neutral and ground- in the sub panel however I see you are a lineman and not an electrician licensed to do this work. Therefore
