I had a panel feeder to hook up today.
A 400amp 3 phase panel feeding a 225amp panel.
400 amp main breaker, with a 225 amp breaker installed in the panel board

The 225amp panel has a main breaker. The 225amp panel is nippled to the 400 amp panel.

I asked the foreman, couldn't the 225Amp panel have been a MLO panel?
He said, yes it could have been because it is "Within sight."
I am unaware of this regulation.
I looked it up for myself and can't find a "Within sight" rule.
 
Right. As long as it's in the same building, the 225a panel could still have been a main-lug panel.
 
Many people wongly assume that all disconnects have some type of "within sight" restriction.

I can only think of within sight restrictions for "motors and appliances" but I am sure there are some others.

Nope. That's the point, isn't it? It is the absence of a code article that would prohibit something that allows us to conclude that that something is allowed.
