I am looking for a definition of two terms on a 25Hp 240V motor. The terms are "Form C" & "Type E".
The motor name plate does not have a NEMA Code Letter.

Form C may be the mounting of the motor. A C-face motor is directly mounted to another piece of equipment (i.e. a gear box) instead of having mounting feet.

Type E probably means it is a very energy effcient motor. The NEC used to have different application considerations for Design E motors.

I think that Jim's correct on the C term but on the E term it could also be telling you the insulation type Class E Class 120 : Temperature is 120 degree C 248 degree F. Just another idea.
