What is the minimum HP motor that is required by the NEC to have a motor starter?

I don't think minimum HP is a factor. I believe it is motor protection. Starters can be either manual toggle switches or puchbuttons, or a magnetic starter any of which must be rated for the motor HP which it controls. The second consideration is motor overlaod and motor circuit protection. Overload protection is often provided by an thermal device internal to smaller single phase motors where it is provided within the motor starter on larger HP motors by the use of specifically sized "heaters" selected to protect the motor being controled.
Motor circuit protection can be provided buy a common circuit breaker or fuse or the started may be used in conbination with a motor circuit protector type breaker.
Bottom line is that there many ways to accomplish the control by realistically it depends upon what is available such as you may be able to get a simply 3ph HP rated 3p toggle switch type control with integral motor overload protection for a 1HP motor where the same configuration isn't availble for a 25hp motor. So simple availability is also a limiting factor of your choices.

Every motor requires a controller. Even a 1/8 hp motor such as an electric clock has a controller.
The type of controller varies depending on the hp of the motor, how it is to be controlled, and how much control is desired.
