So let's say I have a three-phase motor being fed from a 13.8kV delta / 480v wye transformer. The motor nameplate has two different ways of wiring the motor: a high voltage of 460 and a low voltage of 220. I'm confused because the system is 480v line-to-line but 277v line-to-ground, so which voltage do I select?

A good indication that you want to use the line to line voltage in making your selection is that there is no neutral connection to the motor.
If the source were delta the line to line voltage would more obviously be the only one that counted.

Is this an old motor? It's been a long time since I've seen a NEMA motor voltage spec'd at 220V.

Very old. I work at the ADM plant in Peoria, IL. Back in the 30's it used to be the largest distillery in the world when it was owned by Hiram-Walker. We still have switchgear from the 30's.

Obviously we have multiple arc flash remediation projects underway :D
