Mounting Fluorescent Fixtures

In covered parking garages where outlet boxes are recessed and flush with the concrete ceiling surface, is it permissible to install fluorescent luminaires directly over the outlet box using a concrete box cover, chase nipple and locknut; whereby, the fixture itself is then fastened directly to the concrete ceiling surface using approved anchors? This technique has been used widely in the past, but is there any conflict with NEC 410.14 (B) pertaining to "access to boxes"?

No, that is a violation. You must punch a large KO in the back of the fixture to be able to access the wiring with out removing the fixture.
