Mounting height for NEMA4 enclosure
The code requires a maximum height of 6'7" to a breaker handle from the floor to allow for ready access.
If that breaker is within a NEMA4 enclosure that requires tools to open hardware along all edges of the door; would the top edge of the door not have to be within 6'7" of the floor as well? What is the point in having the breaker within reach if you need a ladder to open the door? If there a code section that would support this opinion?
 
This is an industrial control panel containing overcurrent devices along with meters requiring visual observation. Why would an overcurrent device in an industrial control panel not have to be readily accessible?
 
I remember a thread about ANSUL systems. The debate was whether you could put the contactator in a simple 'pull box,' or if you needed an enclosure that was listed as a 'control cabinet.'

The assertion that you needed a 'control cabinet' seemed specious to me then ... but this thread gives me pause. If it's a safety issue, I can see someone requiring either a remote operator for the breakers, or a shunt-trip arrangement. Or, even, the use of a contactator and some push buttons.

If it's not a safety issue - that is, the breaker is not ordinarily used to shut down / disconnect the equipment, I am willing to accept the installation as you describe. Otherwise, it would appear that the wrong enclosure was used.
