MTM-TT-MTM switchgear, who is responsible to develop and program sequence of MTM operation as well as testing it works?
Is it gear manufacturer? Will they do at their facility and test again at site once the gear is installed?

Sequence of operations by the customer's agent. Hopefully they know what they want on and off and when.

Programming by the equipment designer, who may be the customer's agent. Factory commissioning by the programming group.

On site commissioning by installing contractor/sub contractor and customer's agent. Independent testing agencies are usually employed.

It all depends.
Someone who takes the customer's thoughts and turns them into a design.
Someone who the customer designates to speak on their behalf.
May be a contractor, a corporate engineer, the architect or engineer of record, a turnkey construction comany.

The main thing is that the "customer's agent" here represents and is responsible to the customer, not to the other contractors, manufacturers/etc.
