Is there anything in the NEC or manufacturers recommendations that states when a multi-conductor lug is used that all conductors in that lug have to be of the same size and physical characteristics?

Only if it is a parallel circuit.

For example if there is a 1/0 cu conductor and a 250MCM cu conductor under the same multi-conductor lug it is ok?
and if so how would you select an appropriate torque specification for that connection?
