Multiple Equipment Grounding Conductors

I always thought that it was a code requirement that when you had multiple equipment grounding conductors in a device or splice box they were all to be spliced together. For example a 12-2 w/g 14-2 w/g 12-3 w/g. I can't find in the code book where this is required. I have always spliced them together and grounded the device with the largest conductor in the box. Recently I have ran into some installations where each circuit only ties to the equipment ground for that circuit. Any thoughts or code article that says one way or the other is required.

Grounding Conductors to Boxes. Where circuit conductors
are spliced within a box, or terminated on equipment
within or supported by a box, any equipment grounding conductor(
s) associated with those circuit conductors shall be connected
within the box or to the box with devices suitable for
the use in accordance with 250.148(A) through (E).
Exception: The equipment grounding conductor permitted
in 250.146(D) shall not be required to be connected to the
other equipment grounding conductors or to the box.
 
I have went round and round with my supervisor on this subject as well as corrected by Don and Dennis..What i gome across or have to do is for example 3 raceways passing through a 12x12 using a wire type egc and my supervisor has me cut every ground ,,,install a ground bar after i sand the box shiny then tap and thread for the bar and screw to lock washer to nut the bar to the box.
IMO its a non issue to splice all of the grounds because i think the goal is to protect the splice box so why waste the time cutting perfectly good grounds, except the largest in amps,,and that one is going through a mech lug that connected to the only hole they offer in a splice box...One hole is all they offer so they must realize it shouldnt need more than what can be bonded by that hole.
