Where is it required to use the below termination kits?

1. Dead break tee connector termination kits - Where is it used?
2. Load break elbow termination kits - In all pad mounted transformers
3. General termination kits - The ones which are connected to porcelain bushings (Live front)

What are the cost differences?

Dead break TEE connectors.. usually for 250, 600 or 900 amp installations.
you may can find some in 200 amp, but why...

Load break elbow termination kits. Most common, usually 200 amp load break.

live front? Why in this day and age? Keep that for the high stuff over 10 ft high.
they are the same anyway..

cost differences are easily obtained at the supply houses.

What type of bushing wells are typically provided in indoor medium voltage and outdoor medium voltage switchgears for termination?

That’s up to your specifications when you order it.
transformers or switchgear can be ordered about anyway you want it, live front, 200 amp bushing wells for loadbreak bushings or higher amp deadfront equipment.
