NEC 210.4 multi-wire branch circuits

If 210.4 B says that all multwire branch circuits must have a disconnect means so that all their ungrounded conductors simultaneously disconnect

And if 210.4 c says that a a MWBC may only supply line tonneutral loads then why does

210.4 c exception 2 say that a MWBC multi wire branch circuit may supply line to line loads only if all ungrounded conductors are disconnected simultaneously

This last exemption makes 210.c invalid (can only supply line to neutral loads) because MWBS must always have their ungrounded conductor disconnected simultaneously

I kind of wonder if they don't intend the situation with line to line loads to have a common trip breaker and not just a handle tie? Sort of says that but could be worded little better to be clear about this.

Manually disconnected, not necessarily automatically.
And if 210.4 c says that a a MWBC may only supply line tonneutral loads then why does

Automatically disconnected as well as manually.
