Situation: Customer owned 2000kVA XFMR, (7) sets of (4) 500mcm. Neutral is bonded to grounding electrode conductor inside the pad mounted enclosure. Inside the building, at the service entrance point a bare grounding electrode conductor bonding the a main bonding jumper and building steel.

Questions:
* If the system's neutral is bonded with ground at the transformer, is it required to be bonded in the main disconnect panel?
* Should a properly sized grounding conductor have been provided from the transformer to the main disconnect panel?

Yes to first question and no to second question. See 250.30(A)(1) Exception 2. Also see 250.32(B)(1) Exception 2.
