Came across a state job in progress where engineer (PE stamped drawings) specified 12"x12" pull boxes for 3" conduit straight pulled and 3" conduit angles pulls. The conductors installed are 4-350mcm with #1/0 equipment ground.
The cables look like twisted, barber shop sign and seem to be wringed out like an old rag.
I told the lead PE inspector that the cable should be removed and taken off the job and proper boxes installed. They'll probably just Megger test and use them. Thing is, the circuit provides power to a data center processing $100,000 per day. Crazy.
