I am feeding an adjacent building which has an existing 208 volt service from a newly constructed switchgear next door which has a 480 volt service. I am disconnecting the utility service (already met with utility) and providing a 480v/120/208 volt transformer and refeeding the existing service. I am not planning to use a 480 volt disconnect prior to the transformer inside the building since I have overcurrent protection prior to the transformer from the switchgear next door. The 208 volt disconnect is approx. 10 feet from the transformer and 20 ft from where the conductors enter the building? Does this violate 225.32 "Install at a readily accessible location nearest the point of entrance of the conductors"
 
A disconnect for the 480V entrance is required.

If you put the transformer outside, you'll still need a disconnect for the 208/120V entrance. 20ft inside exceeds what most consider compliant, but there is no specified distance. To go that distance you'd have to use one of the "considered outside" methods for services.
 
Unless you happen to fall under one of the exceptions, IMO, you would need a disconnect on the 480 at the building to be in compliance with 250.32-39
 
Since this service is protected by a circuit breaker in the adjacent building would the 480 volt disconnect need to be fused?
 
Yes, I am talking about the 480 volt disconnect in the second building feeding the new transformer. Overcurrent protection exists in the switchboard that will feed this new disconnect.

Just to confirm, no additional OCPD is required. Just a non-fused disconnect switch.
 
Keep in mind that 225.36 does require that the disconnect at the detached building does need to be rated a suitable as service equipment.
As I recall, no-fuse switches are but I would confirm that my disconnect meets 225.36

SUSE may depend on NEC edition in force. 2014 changed 225.36 to require SUSE only for existing [compliant] feeders with no EGC.

SUSE is required under 2011 for non-residential.
