Can anyone explain if neutral or grounded conductor has to be taken for any 3 phase 3 wire system (panel, motors) as per nec

Welcome to the forum. Could you try to explain with a little more detail what you mean by a "taken" grounded conductor?

Past the service equipment and the main bonding jumper, if you don't need the neutral (grounded conductor), you are not required to take it. You are not required to plan for the future but I would take the neutral to any down stream panelboard as a means to feed line to neutral loads sometime later. :)

I have 3 phase 3 wire equipments(208 V), do I have to take a neutral to the equipment's disconnect as per article 200.2 even though the equipment does not require a neutral. If not required can you please explain if do we have any code exempt

No exception by code. More importantly, you want a neutral at the panel and you want it bonded. That is what provides the path to trip cbs for a ground fault.

I'm curious - why would you not want a neutral to the first disconnect?
