what power factor does table 430.250 use to calculate the FLA for the squirrel cage and wound rotor motors?

another question.

does the NEC require certain spare capacity when sizing panelboards or switchboards? or is this just good practice?

I don't think it is a calculation. They describe the numbers as being "typical." That sounds to me like empirical information, not calculated data.

The code does not tell us to maintain a minimum amount of spares. That is a design choice, or an owner's call.
