NEC transformer sizing

Does the NEC specify how to size a transformer...in terms of connected load or demand load?

The code is very clear on how to protect the wiring and the transformer, but it doesn't tell you which one you need to buy.

If you can imagine a time when the NEC really wasn't a design manual this is one of the remaining artifacts.
