Need Dual Wall phone and data jack combo??

Anyone seen a combo Wall phone and Data jack combo?
I think i've seen one can't remember where. It has post for a wall hung phone and a data jack on the side.

Sorry this jack does not have a data jack. RJ-45

You can make them using whatever configuration you want with keystones and plates.

Look up Leviton's Quick Port system. They have a lot of configurations, and may have the cover plate you need. In case you did not know, you just click the needed items into the plate.

Oh as to what I need that for.
Well I have a situation where I have wall hung phones on the wall jack/plate. and would like to have the cat 5 jack come off the side like the plate that was suggested. I will probably just cut in a jack and call it done.
