Can a predrilled interior Nema rated pull box have its holes modified or re-drill new holes. Does this kill the integrity of the NEMA rated box?

Your fitting needs to have the same rating as the enclosure and environment require. As for the number of holes, I don't think I have ever seen any documentation about the number of holes. You'll have to be careful about wire fill 314.16.B, 314.28.A, etc.
