neutral and ground rods

I came across a residential service wheret the ground rods conductor was connected to the neutral lug along with the neutral in the meter box. I thought 1 lug 1 wire ? Is this allowed by most inspectors ? also was there a time when the neutral was bonded in the meter box if so would the circuit panel be bonded too ? thanks for the reply.
 
Nothing wrong with attaching the grounding electrode in the meter - but not in same terminal port as another conductor, unless specifically designed for the purpose. (you probably will not find such a design here though).
