I've seen many of the same thing pass inspection.
So, who is to blame, the electrician, or the inspector?
And the same bar!
Yet still a problem because the grounded and grounding are in the same holes...Not a problem if this is the service panel with the main disconnect.

I see nothing wrong with the neutral bar. Other than the slots are lot all the same direction.
Now, the conductors that are in it, well............

Them terminal are obviously not torqued, becaouse the screwdriver slots are not deformed ????
