Is it now code that it is 1 neutral per circuit be it 120/208 or 480/277? For example, in a 120/208 situation can circuits 1,3 and 5 share a neutral? Where is this stated in the 2008 code? Thanks
 
No. What 210.4(B) says, is that if you share a neutral then the breakers must all handle tied together, meaning 1,3, & 5 would be on a 3-pole breaker.
 
210.4 B does not talk about the grounded conductor, please clarify.

Well, (C) kind of clears that up, but to be a multiwire branch circuit there has to be a grounded conductor otherwise it's just a 220V circuit, or whatever.

And Dennis is correct you could use a handle tie.
 
You can use an individual neutral for each ungrounded conductor, a shared neutral for two or three ungrounded conductors (MWBC) depending on the system or one large neutral for multiple ungrounded condcutors if the installation falls under Article 225.
