I kid you not. I worked in a house today with NM cable from about the early 1960's. Black wire was #14, neutral was #12 & cable had a reduced ground, #16, not unusual for the time. But ground was insulated, which I have rarely seen. I thought I was dreaming but looked at it several times.

Has anyone else ever seen or heard of this? I wonder if the factory tech just picked up a wrong spool of white wire when this cable was produced.

I have seen the equipment grounding conductor like that but never have I seen the neutral and hot conductors of a different size.

Did you by chance see what any marking on the cable said?

I have seen the 16AWG EGC insulated several times before.

I have seen insulated grounds now and then, so that wasn't totally new. Jacket was marked 14-2 w/g, or similar, as most are marked. I'm pretty sure the 12 neutral was a factory error but I have never seen that before. I should have noted brand name but didn't see it & didn't have a lot of time either.
