non-functional disconnect

We have a few pieces of equipment that is 120VAC 1PH 10AMP. The supplier of the equipment designed it and equipped it with a 480VAC 3 PH disconnect. This 480VAC supply is tie into a single phase transformer, Primary 480VAC, Secondary 120VAC. The transformer and disconnect is attached to this equipment.

Are maintenance staff, due to the lack of available 480VAC, has modified the main supply voltage to 120VAC. They did this by removing the orginal supply wires form the terminal and replaced it with a direct Plug-IN-CORD single phase 120VAC. This is a temporary situation.

Should we indicate in some way that the 480VAC supply disconnect is not a functional part of this equipment?

It's a good idea to label it with all kinds of information. In addition to
"disconnect is non-function",
you might add,
"beware - product has been modified from its UL tested configuration. approach at your own risk"
