I did, many moons ago, for our first color TV. When we added an antenna rotor and a UHF converter, my dad bought one of those and let me "help" him install it.

Is it for sale?

When I was in code school I started a collection of old random devices that we pulled out of various old homes in MA...Many homes are over 100 years old so you can imagine some of the stuff I found.

Made for a good show and tell day near the end of the class.

Haven't ever installed one, but I've seen them.

They also made triplex's

That was made by the now defunct Sierra Electric, the wall plate was their "decorator" style.

That triplex can't be nearly as old....when did they start making receptacles with the slot for the grounded conductor longer than the slot for the ungrounded conductor?

Not sure when they started making them, but the interchangeability language of 406.7 seems to appear first in the 1975 NEC under 410-56(e).
