What is the biggest mistake you have made on a job-site?(Explosions,fires,electrocutions,etc.)

In my case it was rushing to much and not paying attention to my surroundings, on one job we had 120v and 277v lighting, me and my partner installed six 120v 4 lamp lay-ins in a small break area, much to our surprise (when we turned on the switch) was a loud hum and lots of black smoke! ..........Yes you guessed it, 277v to 120v ballast, needless to say we were not voted employee of the month, o yea lucky for us the sprinkler system was shut off for alterations.
