what is the requirement for receptacles feeding off a isolated transformer in a o.r. example orange, green triangle.

See article 517.160, and if you're under the 2008 take note of the "stripe" requirement.

BTW, a triangle doesn't come into play, you are probably thinking about the green dot identifying a HG receptacle.

What was the reason for the 'stripe' requirment being added?? I can only 'assume' that this was give the 'isolation' system its own 'distinct' color, so people won't think its a 480/277 system.

The green dot identifies it as a Hospital Grade device. The triangle indicates it has an Isolated Ground, many of the IG types are also orange colored.

You might want to take a look at 517.19 and 517.30(E) for recep. identification. I use red plates with EMERGENCY engraved. These should be readily available or on short order.
