Opinion On 300.4(F) - Insulated Fittings

Does a Myer's hub with the insulated throat require a plastic bushing under this article or does it qualify under "...providing a smoothly rounded insulating surface, unless the conductors are separated from the fitting or raceway by substantial insulating material that is securely fastened in place."?

Maybe he's incompetent. Did he cite a code section that requires something beyond the "substantial insulating material that is securely fastened in place"?
