Please see attached sketch and read it with following:

Would intersystem bonding termination mounted outside on building wall beside the main service disconnect have conductor from the termination directly connected to the main service disconnect enclosure acceptable?

Yes, they actual make ones that mount directly to the outside of the meter base.

But what I have is from the termination conductor #4 connected directly to service enclosure. The termination otherwise they conductor is NOT attached to meter or the service disco. Is it unusual to see this see sketch post#1?

250.94(4): “at the service equipment, be securely mounted and electrically connected to an enclosure for the service equipment, to the meter enclosure, or to an exposed non flexible metallic service raceway, or be mounted at one of these enclosures and be connected to the enclosure or to the grounding electrode conductor with minimum 6 AWG copper conductor”

Yes but I have seen it on meter enclose, conductor to meter enclosure but bonding conductor to main service enclosure I have not seen. You have to manually place lug, screw to enclosure? Does service equipment come with lug or something to be connected to enclosure?

This is following an all too familiar pattern ! You asked a question and have been provided Code wording that directly answers your question.
It is absolutely ridiculous for anyone to have to explain how you attach a conductor to an enclosure.
