The tap conductor is wrapped to the wrong direction on the screw. Also that looks like a metal framing screw. A listed screw for this purpose would be required.
 
Think of all the money we would save it we just ran a couple big wires into a box and then screwed all the branch circuits into them. I think this guy was on to something.
 
Looked like the violations had been covered, but found one more.

The blue wire nut is not listed for 2 #14. (or #12)
 
Total hack work...everyone knows you remove the main feeder (live, of course), clip off a wire or two, stick it back in with the #12 under the lug.

The CCW wire wrap around the screw is correct if it's a left handed screw, or in the Southern Hemisphere. He should have put No-Lox on it tho. :slaphead:

eta: joking, sarcasm, and incredulity aside, the person who installed that took an awful risk to make a very non-compliant install. It would be ironic if that panel had breakers rated for two wires load side. Pigtailing two wires to one breaker would have taken less time and been infinitely safer.

Saddle valve... that way the installer could throttle the flow of electricity as well. :roll:

Another sign of hack work: a 'pro' would have tapped the line side of the meter. :angel:
 
I don't see arc-flash protection in place :blink: Neat and workman-like manner?

If the rest of the place looked like this photo, the inspector would have carpal-tunnel syndrome writing up all the violations before getting to 110.12, tho 110.12(B)(insulation damage) would be on the list somewhere.

Arc flash protection was probably via a thirty year old pair of Oakley sunglasses from a dollar store.

As the resident plumber on the Code forum, you're very welcome! Reminds me of a time... I had someone working for me when I was with my previous employer that wired up a whole bunch of receps. with the wire looped around the screws CW, like in the pic. One came loose, and he was admonished, but I got into a bunch of hot water for my admonishment.
