Outlet box in concrete form walls

Ok, first time ever post on any forum.
I am looking for a ways to support 4sq boxes in concrete form walls for devices. It has been years since I have worked on a concrete form/deck job so I might be a little out of touch with modern industry standards. What are electricians using to day?

In the past, I used, and I can not remember the name of the form anchor for the 4sq box. What is its industry name and who makes them? Are they still in use? I have had no luck with suppliers or my expeditor in locating these anchors.

Description: Washer type assembly with 1/4" thread, washer assembly attaches to back of 4sq box through the center ko and is held in place with two (for lack of a better name) tits, that would hold the box in place through two of the mounting holes in the back of the 4sq box. 1/4" rod would thread in to the washer assembly. Drill hole in form and install washer and 1/4" nut on the face of the form wall.

Alternate method B:

is how I do it, without much difficulty. A short piece of wood on the outside of the form between the ply and the tie wire, rotated to tighten up the tie wire, which pulls the box tight to the forming so that no concrete penetrates during pour.

Thanks guys,
The "no bolt" is on the right track but will not work in the form wall. Tie wire is one of my other options. Since I can not find the device I described, I might try 1/4" rod through the monting hole and 1/4" fender wahers and nuts to sandich the box to the form.

Was the no bolt fixture stud in post #2 what you were looking for?
 
Don't forget to duct tape your boxes if the concrete is going to be on the wet side. If your really worried, fill your four square boxes with sand before you tape them, then you know they'll be good no matter what :D
