Have a 120volt/208 panel and single pole breaker that feeds a 20amp 125 volt twist loc outlet. It reads 211 volts from hot to ground and the same across hot and neutral. The neutral balances the load? It works fine, how can that be?

Recheck you supply. It sounds like you may be supplying it from a 240/120 Hi-leg panel.

That would be my guess as well, the supply is a 3 phase, 4 wire Delta system.

What works fine? Equipment plugged into the receptacle?

As others have suggested, you probably have a 120/240 high leg delta (B phase) feeding that receptacle.

I know of an entire apartment complex that got erroneously wired this way (transformer was 240V delta high leg and the electricians thought it was 208/120V). Oops.

I can 1 up you. I had a guy years ago that wired a tanning bed this way. It did not end well. His first comment after was, gee, I wondered why every third space was not used in the panel.

Most tanning beds fed by a 120/208v service are actually boosted to try and maintain a solid 220-230 volts.

Use the wild leg all the time in 3ph 240v delta panels to feed 240v tanning beds loads

Odd he had any trouble at all.
