Are there requirements in the NEC that dictate the height of overhead powerlines, cable TV Cable, and phone lines above streets, alley ways, drive ways, side walks, etc?
Yes there are but those rules are for customer owned lines. The NESC covers most of the lines you are talking about since they are most likely utility owned. Most is appropriate since some states adopt the NESC into law, some do not, and others (like California) have their own set of standards they enforce.
