Overhead service conductors

For services to dwelling units, isn’t the utility responsible for bringing conductors to a service mast? So for the clearance requirements listed in the NEC for overhead service conductors in 230.24, when does this section apply?
 
As you said most the time utility is responsible for these conductors and therefore the service point is generally after these conductors.

It would apply any time the service point is on the supply end of said conductors and there is some occasions where that is the case.
