Pan cake blanks

Have you ever heard of a pan cake blank, and how would it be used in a LO/TO situation such as this language describes below. This is from a proposed ANSI construction standard on LO/TO:

2.8 Energy Isolating Device

A verifiable mechanical device that physically prevents the transmission or release of energy, including but not limited to the following: a pan cake blank, a manually operated electrical circuit breaker, a disconnect switch, a manually operated switch by which the conductors of a circuit can be disconnected from all ungrounded supply conductors and, in addition, no pole can be operated independently...

Could be a blank plate designed to insert in the flange of a compressed air, steam, or water line to positively seal off the work area from mechanical sources of energy. LO/TO applies to all sources of energy, not just electrical. I have investigated fatalities where trapped mechanical energy killed maintenance workers.

That is exactly what it is. Most of them have a handle to identify the "lock out" point. The flange bolts are loosened and the "pancake" plate is inserted, then the bolts are tightened down. This is only done after the valves are closed and the system relieved of any stored pressure.

Thanks for all the posts. I hadn't heard of any use of a pan cake in an electrical setting.
We use the Pan cake blanks in our steam lines, coal feed lines, and water and air lines.
But didn't want to put any ideas out there before hearing fresh views.

pan cakes are the thin round electrical light boxes, and pancake blanks are just light box blanks, two sizes.
