Panel height

I have been studying for an exam in Pittsburgh and I had a couple or of questions I could not find answers to.

1. For Panel box location in a house is there a max or minimum height, I know in 110 there is alot dealing with working space but I could not find box height specs.

2. Also in panel boxes I have seen many times panels neutral/gnd bars with neutrals and ground connections with more than one wire under them. I do not use this practice but is there a code reference stating that this is prohibited.

Thanks as always and any prep book suggestions would be helpful here in Pittsburgh they only recognize one test the Master Electricians test.

Question:
1. see 404.8(A)
2. see 408.21 for grounded (neutral) conductors, grounding conductors are left up to the manufacture and will be included in the listing and labeling, so look at the label in the panel.
