Panelboard Schedule Load Calculation

Can someone please share with me panelbpoard schedule for three phase 208/120 with calculation, has 208 3 phase load, 208 1 phase & 120 1 Phase?

I use the same schedule for 1Ø and 3Ø panels. You're kidding yourself if you think you're doing something useful by balancing the loads on a bunch of receptacle circuits.
