Where in an electrical system is it required by code to phase your conductors for the proper nonimal voltage present? I.E. #12 branch wire in BCP, Fusible Motor disconnect, MCC bucket breaker?
 
By phasing I mean the color of the wire. Is it necasary to phase all wire regardless of size or what it is feeding? Or are there certain instances where it is not required like on small branch circuit wiring?
 
After reading this code reference, I get that the branch wire must be colored based on the nominal voltage. But is it legal to install a Red wire on A or C phase, since it is colored for the nominal voltage? What if someone only had Red wire and used it on all three phases on a 3 pole breaker in a 120v panel?
 
Yes unless it's the Neutral or EGC, for these two conductors see 200.6 and 250.119.

The code rule requires identification by phase and system, so you can't use the same color for all 3 phases. (note that this is assuming that the building has more than one nominal voltage system, if there is only one voltage system there is no identification rules for the ungrounded conductors other than they can't be white, gray or green)
