Portable Outlex Boxes

What are the NEC / OSHA regulations that apply to portable outlet boxes. I have a client (body shop) using what appears to be a home-made (metallic caseoutlet box to power a portable welding machine. I am reasonably sure that this is not allowed but want some expert advice.

Regarding the box itself. It is required to be secured. 300.11 / 110.3(B)

Regarding the cord connector. It is required to be listed for the application. I am sure that two screw NM connector is not listed for a cord of that size if it is listed for cords at all. 300.15

Regarding the tapped up cord. The cable sheath has to reach the box. 300.12
 
314.23.(H)(1):

(1) Flexible Cord. A box shall be supported from a multiconductor cord or cable in an approved manner that protects the conductors against strain, such as a strain-relief connector threaded into a box with a hub.
 
OSHA does not allow that kind of box. Osha was just at my shop and we had one of those boxes hanging from a cord pendant for temporary power.
