The arc flash can be created by insulation failure, even though there are no exposed live parts, a nick in the wire, a deteriorated buss insulator or any other number of things can, and has created arc flashes, the movement of a wire or a breaker can cause a fault to ground. If the cover is off, the flash may not be contained, in which the proper PPE may reduce the chance of injury.
