Preferred floor receptacle in poured concrete slab

I will be roughing in a few floor receptacles in a slab soon and wondering if anyone has specific products they have used and like?

The box is recessed beneath the floor surface so it can be used like an In-use cover. I thought that was clever.

Slab will be finished floor.

We have used many of those boxes in the past. On the last go around we used a Legrand Omnibox product. They can be much cheaper than the FBS products. A lot of this comes down to what's going in the box and what style of cover you will use. The prices can shift dramatically if you choose something like a polished brass cover.

 
We usually design around the Legrand Modulink 880MP plastic boxes. We like them because they are the only floor boxes that we know of that accepts (2)2" and (2)1-1/4" conduits and can be linked together (up to 3) under one cover.

I usually spec. hubbell system one.

There are a lot of options no matter who you go with -
flush receptacles or recessed like the ones you mentioned,
on grade weatherproof box, or above grade box,
plastic or metal boxes,
square or round boxes and covers,
# of gangs,
dual services with comm., or just power....
type of flooring
