Primary Disconnect Switch Required?

If I have a 480/208V transformer installed above a lay-in ceiling, I'm I required per code to provide a non-fuse disconnect switch on the primary side even though the primary conductors are protected by a circuit breaker in a panel that is not within site? If so, does the disconnect switch need to be located within 6'-7" AFF or can I mount in adjcent to the transformer above the layin ceiling?

It's a grocery store and the 480 Volt panel that is feeding the transformer is on the other side of the building. The primary conductors are protected by a circuit breaker in the 480 Volt panel. The 208 Volt panel has a main cirucit breaker.

A local primary disconnect is not required. Having one makes maintenance easier, but that is a design issue, not a code requirement. If you do install one, it need not be with 6'7" of the floor.
