If you look at the Exception to 314.28(A)(2), it will refer you to Table 312.6(A) when your conduits enter opposite to a removable cover.
In you case, I believe that dimension is 4".
 
No matter which or what set of rules you use ,you still must comply with

I disagree. Art. 314.28(A)(2) clearly states to use 312.6(A). IMO that overrides 300.34
 
Off the top of my head, we may be mixing apples and oranges.
300.34 references cables over 600 volt. Although I don't notice a voltage limitation on 314.28, there is a separate section for cables over 600v (314.70 & .71) which does reference 300.34
