Is there a code that requires a Pull Box between a certain distance?
If you have an extra long pull, you may have insulation damage if it is overstressed. If that is the case, you need to do the calculations. You might also read FPN 1 and 2 to Table 1 in Chapter 9.
FPN No. 1: Table 1 is based on common conditions of proper cabling and alignment of conductors where the length of the pull and the number of bends are within reasonable limits. It should be recognized that, for certain conditions, a larger size conduit or a lesser conduit fill should be considered.
The bolding is by me.

a good rule of thumb is not to have a longer conduit run than the longest fishtape you own, even if you are using a mouse blower its nice to be able to push a fishtape if you have problems with the mouse... if I run an exceptionally long underground run , I pull the string in as I run the conduit, and if Im using PVC, I like to use ridged sweeps for long pulls so the string doesnt cut the inside of the radious.....

That could add up to a lot of boxes for some of the contractors I know.

I like to have pull boxes for long runs where the conduit is exposed, underground its alittle less practicle, thats why I pull the line in as I run it, or at least blow the line in before I backfill...
