Is there a specific requirement for pull box locations for underground feeders; ie is there a maximum pull distance or is it strictly governed by number of bends?

As far as the NEC is concerned no, you can place the boxes as far apart as you want.

There are some practical considerations for how far you can pull with out conductor damage.

If you are running PVC and have to use some 90 or 45 degree fittings be sure these are RMC sweeps, better still large radius ones.

Best avoid using bends at all if you can.

To: pgull and iwire, I should keep quite, but I feel that I need to pass along this information.
Many years ago, I had a 600 volt three-phase service with three-500 mcm thwn cables to install.
I was new in the game, and the head electrical engineer told me, how we would do the job, Bottom line, I listened to him and I am thankful for that.
The conduit run was 400 feet, and it had four 90 degree bends.
The NEC Annex C,Table C1 states that you can put three 500 mcm thwn in a 2-1/2 inch conduit.(My advice do not do it). Table C1 also states that you can put five-500 mcm thwn in a 3-inch conduit.
(My advice again, do not do it).
The electrical engineer said, Bob, I would run 4-inch conduit, and at each 90-degree bend, I would have a 2-foot x 3-foot x 8-inches deep junction box installed.
I listened to him, and here is were experience comes in.
As we are all aware the NEC is just the minimum requirements. If we want to up-grade the service, so be it.
