I have a project where I called out upgrading an existing meter/pullout in a Square D "three pack" from 100 amps to 200 amps. The meter base was already rated 200 amps. The pullout is Boltswitch and I have no issues there. The issue is with the conductors between the meter base and the pullout. My notes required use of Square D parts. The contractor field wired. The inspector has withheld approval.

Is field wiring like this typical and acceptable? I'm being asked to approve the installation.

So the contractor changed the factory conductors between the meter and the switch? IMO you would need to ask Square D if that equipment modification is permitted and if it will still maintain the listing of the equipment.

Thanks. I agree, but I wanted to see if there was an accepted trade practice that would allow field wiring.

If everything is rated for 200 amps and the new conductors are terminated properly then it's likely that nothing will ever happen to the installation. Just a question of the listing and who will sign off on it. If Sq D said that the conductors are field upgradeable then everyone should be happy.
