PVC conduit and bushings

I have a question about 352.46. I did a quick search and did not find anything so forgive me if this has been discussed before.

Is a plastic bushing needed when entering a box with an MA (for wires smaller than 4AWG)? I have always had it interpreted it this way but today someone pointed out the wording of the article and it reads that if the conduit enters an "adapter" it does not need a bushing. If a PVC connector is an adapter ("MA" or "TA") than does that mean that that entry does not need a bushing?

Again, I've always heard it explained that you need a bushing when using PVC to enter a box but interpreting the adapter to be the connector it seems to contradict that. What do you think?
 
352.46 "unless the box, fitting, or enclosure design provides equivalent protection."

There was a proposal to require a bushing on PVC MA (I think 96 or 93 Code) The CMP reponded a bushing is not required a PVC MA provides equivalent protection.

The way I have been tought, PVC manufacturers do not make "connectors" they make Male Adaptors and Female Adaptors and couplings. Why, I don't know..1st base.
