PVC Conduit

In Chapter 9, Table 4, there are four Tables listed for PVC Conduit, One is Sch 80, One is Type EB, One is Sch 40 & HDPE and One is Type A, Sch 40.
Which Table applies to the "standard" Sch 40 normally used in above ground installations ?
 
Actually this was a "pass along" question from an "lurker" and the person initiating it was using Table 4 since it was conductors of different sizes.
If I recall correctly, Type A PVC is listed for concrete encasement but I wanted to confirm before I answered him.

Now that first part.... that's going to take some time. :lol:

Sch 40& HDPE---The applicable table is the one that states art. 352 and 353-Rigid PVC Schedule 40. No other table states sch 40
